Dermatology Specialists has been providing quality care for all of our patients since 1987. We welcome new patients to our practice! Please see the information below regarding preparation for your appointment, forms, and the insurance companies through which we are participating physicians.

Preparing for Your Appointment
For all appointments, please plan to arrive 15 minutes prior to your appointment for electronic medical record processing.
Bring With You:
- Photo ID
- Insurance card
- Patient paperwork filled out (new patients)
- Medication list
- Allergy list
If coming for a skin check, please plan for us to check your skin. Please avoid make up, nail polish, jewelry, intricately sprayed hair styles, and pantyhose that may hide potentially concerning lesions. If unable to avoid the above, please be prepared to remove them before your exam. Referrals: If your insurance requires a referral to see a specialist, please request it from your primary care provider and allow sufficient time for processing before your appointment. Most providers require 48 hours notice for referrals.
Forms: New patient paperwork and forms may be downloaded and printed below. Please be sure to print both the “New Patient Registration” and the “Patient Medical History” form. Minors: Please remember that patients under the age of 18 yrs must be accompanied by their parent or guardian at their first visit, subsequent unaccompanied visits will require a signed consent.

Cancelling your appointment
If you need to cancel your appointment please give us 48 hours notice. You may leave a message on the office answering machine if you need to cancel after the office has closed.
